[PATCH 3/3] blockdev: turn a rw semaphore into a percpu rw semaphore

blockdev: turn a rw semaphore into a percpu rw semaphore

This avoids cache line bouncing when many processes lock the semaphore
for read.

Partially based on a patch by Jeff Moyer <jmoyer@xxxxxxxxxx>.
