Identify failed device from device map & multipath -ll

Hi All,
I have 2 paths to a device and one path becomes faulty.
How can I identify which is the failed device using command line (Eg: /dev/sda)

360a98000572d54726c5a6a74544c684d dm-2 NETAPP,LUN
[size=200M][features=1 queue_if_no_path][hwhandler=0][rw]
\_ round-robin 0 [prio=10][enabled]
 \_ 1:0:1:0  sde 8:64  [active][ready]
\_ round-robin 0 [prio=0][enabled]
 \_ #:#:#:#  -   #:#   [active][faulty]       ====> the device name is not mentioned here
