Hi Alasdair,

I have made the changes for printing device name with the log messages where ever it was needed. As dm-mpath.c does not have access to mapped_device structure, I had to add one new function dm_target_device_name in dm.c to get the device name. Also I have added #ifdef in dm-mpath.c such that dm_target_device_name function is called only in case when CONFIG_DM_DEBUG is set in .config. Current implementaion needs developer to add device name in order to print it as I didnt find any consistent way to print device name(by adding in macro). For example :

Following messages prints device name as "253:12"

where as in ctl_ioctl function, device name is available with respect to its WWN name and I didn't find way to print in major/minor. Please correct me if I am wrong here. Also if you have any better thoughts then please let me know.
