Use blk_stack_limits() to stack block limits (including topology) rather than duplicate the equivalent within Device Mapper. dm_table_set_restrictions() and check_for_valid_limits() must still track the queue_limits structure's members. To that end, physical_block_size and other topology limit changes were added.
