>From a first look, the header looks good. I take it cryptsetup works, but does not recognize your password? If so, please run the keyslot-checker from the source package. If you have cryptsetup >= 1.6.0 installed, it should be enough to call "make" in misc/keyslot_checker/, and then run chk_luks_keyslots from there, but you may also have to install the full package. For damage due to partitioning, the results should reliably show whether a keyslot got damaged. In that case it will find the partition sector in there somewhere.
