1) RHEL version
2) gfs* packages version
3) kernel version

You can also attach cluster.conf for us.

Also if you have valid support contract with Red Hat,please open case via https://access.redhat.com so we can work on this issue in more details where you can provide us all necessary information we need in order to debug this
