RHEL 4: quorate or inquorate

Hi, all:

 

We saw a rare situation that a 3 node cluster has 2 nodes offline, but it maintains quorum.  Is there anyone has any idea how it could happen and what/where I should look into.

 

Here is the output from clustat on one of the nodes:

We are on REHL 2.6.9-34 (update 4).
