How did you copy that file?

cp tries to detect a sparse file but may have failed here.

Try "cp --sparse=always /mnt/rbd/Win2003-002-198-bak/Win2003-002-198-bak_5-flat.vmdk /mnt/cephfs/Win2003-002-198-bak_5-flat.vmdk".

AFAIK this has nothing to do with CephFS as filesystems do not automagically "thin provision" files.
