OK, this is the state preventing ceph-volume from recreating the OSD.

ceph osd destroy is purely an operation at the mon level (which will
update the osdmap) and thus the PV will not get in the way. In fact,
that stale PV shouldn't cause any problems other than perhaps
confusion to those looking at the node. Rebooting the node will get
rid of the stale PV, but you may also be able to manually remove it
via pvremove; I find lvm pretty fickle when it comes to cleaning up
stale state, partially depending on the age of the OS.
