On all OSD nodes I'm using vm.min_free_kbytes = 4194304 (4GB). This was one of the first tunings on the cluster.

Just for grins, what is your vm.min_free_kbytes setting?

Depending on the size and number of your OSDs and your workload, 2-4GB is a starting point.
