Is there any errors before ofter this? Something about a transport or
hardware error or something about SCSI sense?

What does "iscsiadm -m session -P 3"

report? Does it report failed for the login or device states?

If logged in ok, can you just do a

sg_tur /dev/sde

?

On the target side are there errors in /var/log/messages or
/var/log/tcmu-runner.log?
