Just for the record in case someone gets into this thread. The problem related to ceph-mgr beeing started on another host than mgr active one was because python-routes package was missing. Im using Ubuntu 18.04 on ceph hosts and installed ceph-mgr nodes using "ceph-deploy mgr create hostname". Seems like a package dependency is missing.
