I would upgrade to 12.2.12 and set the following:

[osd]
bluestore_allocator = bitmap
bluefs_allocator = bitmap

Just to make sure you're not hit by the "stupid allocator" behaviour,
which (also) might result in slow ops after $period of OSD uptime.
