I've also seen this behavior sometimes (on real hardware without VMs
or Ceph involved).


Somewhat related: please don't use legacy VirtIO block devices
(virtio-blk), they suck for various reasons: slow, no support for
TRIM, ...

Use a VirtIO SCSI controller instead
