I have redeployed the cluster adding all 3 monitors from the beginning
It seems that the proper procedure is to have the correct ceph.conf ( with correct fsid and all monitors specified in mon_initial_members and mon_hosts )
deployed on the future monitors and THEN run ceph-deploy mon add
It might be useful to ceph users if someone can confirm

monmap. somehow one of your monitors has a different monmap from the
others, or, at least, a different fsid.
To confirm if that's the case, try extracting the monmap from one of
those monitors and compare it with the monmap from an existing monitor.
