What version of Ceph? And could it be that the cluster has old CRUSH tunables? When was it installed with which Ceph version?

I'm not sure it even takes old tunables. With half the weight in one bucket (that last host) it's going to have trouble. Assuming things will balance out when the transition is done, I'd just keep going, especially since the three acting replicas are sticking around.
