It looks like you're running the wrong fastcgi module.

Looks that that was the issue:

Now I reinstalled the Ceph patched apache2 and fastcgi module () not
sure if needed to do apache2 as well):

Now that I've got that working I'll look at getting a more complex setup

Just for the record, using these apache and fastcgi modules seems to be the story - I've managed to run through the more complicated examples:

- zones in different ceph clusters
- zones in different regions

... and get replication working (on Ubuntu 12.04 and 14.04 with Ceph 0.87). Thanks for your help. I have some further questions that I'll ask in a new thread (as they are not really about 'how to make it work').
