This looks like a bug to me, I just replicated the problem.

Right now we are in the process of migrating a few of the remote tasks for ceph-deploy into a structure that will let us know what is exactly happening on the remote end but this is not the case yet for `mon` actions.

I've created a ticket [0] to get that done and see if we can tell better why is it that `/var/lib/ceph/mon/{cluster-name}` is not being created.
