Up until 2002, I lived in San Jose CA (Silicon Valley) and due to the
distance to the CO, only had the option of modem (no 56K due to line
quality - I usually got about 23kb) or IDSL which is 144Kb/144Kb for
$140 / month + $400 install. I had the IDSL, but work paid for it :-) No
cable modem service either.

You don't have to be rural to be unable to get affordable high-speed internet.
