I did some more searching and stumbled across a way to make the stepper arrows 
show up.  It is another file:

~/.config/gtk-3.0/gtk.css

.scrollbar, scrollbar { 
   -GtkScrollbar-has-backward-stepper: true; 
   -GtkScrollbar-has-forward-stepper: true; 
}

On my Fedora 28 system this makes up and down arrows which are VERY small. 
They are there and they work.  I bet more fiddling with this file could make 
them larger.
