In CentOS 3, look for mrtg.conf in /etc/httpd/conf.d. This file configures /mrtg for localhost access only. Change the 'Allow from' line to something sensible and it should work. That drove me nuts for a while until I found it. I did read that before, but overlooked it. If using CentOS-3.x and httpd from there then selinux is not available.
