At the end of the log it looks for hda28,

/dev/hda28: open failed: No such device or address
/dev/hda29: open failed: No such device or address
/dev/hda30: open failed: No such device or address
/dev/hda31: open failed: No such device or address

Once upon a time one could more create more than 15 partitions on one
drive (0 through 15=16 parttions at ssum). In case you needed more
there was a specific proceedure until 30 (if I remeber correctly). Can
this be your case ?
