Release Notes: This release includes updates for all vaccine codes except core Vaccine Information Statement (VIS) codes. In addition, the COVID-19 US Vaccine Codes Table has been updated to reflect changes in this release.
Release notes are designed as a guide to highlight changes that have been made to the vaccine codes since the last publication. They should not be used as the primary source of code information. The vaccine code files themselves should be utilized.
General Note Regarding This Release: Codes with "Last Update" dates from April 13,2022 through May 23,2022 reflect the changes and additions in this release.

COVID-19 Related Code Updates
COVID-19 vaccine codes and crosswalks are provided in anticipation of potential vaccine availability under an approved Biologics License Application (BLA), Emergency Use Authorization (EUA), or as a potential vaccine submission for EUA (Pre-EUA). Codes will become effective for US vaccine administrations only upon EUA issuance and/or BLA approval of COVID-19 vaccine(s) by the FDA. All CVX codes are associated to the new Vaccine Group "COVID-19." CPT codes shown are product codes. CPT administrative codes for doses are available on the AMA website. CPT product codes are added as the AMA approves and makes them available.

- BLA Licensed SPIKEVAX COVID-19 Vaccine, Adults 18+ yrs – NDCs and Crosswalks Added. Moderna SPIKEVAX labeled product is expected to become available in the coming weeks.

(*) The BLA license presentation is not currently licensed for booster (half dose). This vial is authorized for primary series (full dose) only at this time. The CVX code associated to these NDCs is mapped to two (2) CPT codes as noted above. For safety, it is important to consider the maximum number of punctures to the vial cap should not exceed 20. CDC initially listed the vial volume as 5.0 mL in the same manner as the EUA licensed product that is authorized for both primary series and the booster (half dose). CDC is actively updating the code set presentation to reflect the 5.5 mL to reflect the new SPIKEVAX label to support primary series dose only.
- EUA Authorization Pending: Pediatric Vaccine for Ages 6 mo to < 6 yrs – NDCs Added, New CPT to CVX Crosswalk Added. Product availability is expected shortly after FDA authorization is received.

In addition, statements from Pfizer and Moderna have been updated for the vaccines that are not currently planned for manufacture. The Excel downloadable file version of this table provides the most readable view for the Pfizer and Moderna statements.
