VIS Document and Code Changes Updated February 1, 2022


Release Notes: This release covers updates for Vaccine Information Statement (VIS) codes and maps only. A separate set of release notes and files are prepared for other vaccine code updates. 

These release notes address code updates for the following categories in two separate sections:
  1. Vaccine Information Statement (VIS) Code Updates
  2. COVID-19 EUA Recipient/Caregiver Fact Sheet Code Updates
The following three VIS downloadable code set files have been updated on the CDC website on 02/01/2022 to reflect the changes in this release:
  • VIS Barcode Lookup Table
  • CVX to VIS
  • VIS URL Table
1. Vaccine Information Statement (VIS) Code UpdatesVaccine VIS codes are created to enable provider systems to implement VIS scanning and to help automate capture of the specific VIS document edition given to the recipient or caregiver.  The VIS codes may be published in advance of the actual VIS document publication, but they are only in effect and useable starting on the edition date of the VIS.  The URLs included in the code set will not point to the new edition until the VIS document edition date. This release includes the following VIS related code updates:
  • One new combined VIS document, “Pneumococcal Conjugate Vaccine (PCV13_PCV15_PCV20) VIS” that replaces the prior “Pneumococcal Conjugate Vaccine (PCV13) VIS” with new codes and new URLs.
  • One new VIS edition dated 02/04/2022 for “Recombinant Zoster Vaccine VIS” dated 02/04/2022 that replaces the prior edition dated 10/30/2019
In the downloadable files, each new VIS has a new document “VIS Fully-encoded text string,” an updated “Edition Date” of 02/04/2022, and an “Edition Status” of Current.  Any previous VIS documents will have the “Edition Status” set to Historic.  Current and previous editions of the same document will retain the same “VIS GDTI Document Code” and the URLs will remain the same across editions.  New VIS documents are assigned a new VIS GDTI Document Code and new URLs.  CVX codes associated to each new VIS or VIS edition are updated from the prior editions where appropriate to reflect current CVX codes. 

The updated VIS document(s) are shown below for reference.  Additional code details can be found in the VIS code set files listed above.

(*) Note: VIS Document Type Descriptions in the VIS vaccine code set may not be exactly the same as the final published VIS document title.

2. COVID-19 EUA Recipient/Caregiver Fact Sheet Code Updates

The CDC VIS Code Set incorporates codes for COVID-19 vaccine Emergency Use Authorization (EUA) Recipient/Caregiver Fact Sheets.

CDC-authored VIS documents will not be available for COVID-19 vaccines while under EUA.  Under EUA, the FDA requires a vaccine-specific Fact Sheet for Recipients and Caregivers (EUA Recipient/Caregiver Fact Sheet) be provided to vaccine recipients or their caregivers (similar to Vaccine Information Statements for licensed vaccines).  Each EUA Recipient/Caregiver Fact Sheet is developed by the manufacturer and authorized by the FDA.  The COVID-19 vaccine related codes in this code set release are provided in anticipation of potential vaccine availability under EUA.  If the vaccines are not authorized, these codes will be retired.

On 1/31/2022 the FDA updated both the EUA Provider and Recipient/Caregiver Fact Sheets for Moderna  on the FDA website following the FDA’s approval for Moderna’s product under BLA license.   The CDC codes for the EUA Recipient/Caregiver Fact Sheet have been updated to reflect the new edition date.
  • COVID-19 Moderna EUA Recipient/Caregiver Fact Sheet - 18 Years and Older: 
    • Fact Sheet now references Moderna’s BLA licensed product and Spikevax tradename

The CDC VIS Code Set has been updated to reflect the EUA Recipient/Caregiver Fact Sheet changes:
  • COVID-19 Moderna EUA Recipient-Caregiver Fact Sheet. New Edition Date 
    • The Edition Date has been changed to 1/31/2022
    • The Last Updated date has been changed to 1/31/2022
    • The Barcode string has not been changed
    • The URLs have not changed
    • The CVX Code has not changed
The above updates impact the following two VIS code files and tables:
  • CVX Mapped to VIS
  • VIS Barcode Lookup Table
Important Considerations:  The CDC is working to facilitate documentation of having provided an EUA Fact Sheet for Recipients/Caregivers in electronic medical records/immunization information systems (EMRs/IISs).  As such, the VIS code set is being used to provide the EUA Recipient/Caregiver Fact Sheet codes for EHRs/EMRs for use in a manner analogous to the electronic system and workflow documentation of VIS.  As with VIS documents, the COVID-19 Vaccine Recipient/Caregiver Fact Sheets will incorporate the CDC’s scannable 2D barcode in the Fact Sheet document.  The numeric string for a CDC 2D barcode is included for each EUA Fact Sheet record in the data code set for this release.
