Pertussis surveillance worksheets have been revised and added as components of Appendix 11. These worksheets are designed to assist with collection of the surveillance data associated with the pertussis HL7 message mapping guide (MMG) for electronic notification to CDC. The 2009 worksheet is presented without annotations, for routine use, and with annotations showing HL7 MMG data element (DE) identifiers that apply for easier mapping into surveillance systems. In addition, the November 2019 worksheets incorporate the additional data elements that are included in the HL7 MMG, one without annotations and one with annotations, again for routine use and for easier mapping into surveillance systems.
