In July 2014, CDC assisted the Ministry of Health and Social Welfare of Liberia in investigating a cluster of five Ebola cases among U.S. and Liberian health care workers who became ill while working at an Ebola treatment unit, an adjacent general hospital, or both. This report summarizes their findings. No common source of exposure or chain of transmission was identified. However, multiple opportunities were identified for possible transmission of Ebola virus to health care workers, and none of those opportunities were found to be a previously unrecognized mode of transmission.

Among the five West African countries affected by the Ebola outbreak, Liberia has had the highest number of cases and deaths. CDC worked with the Liberian Ministry of Health and Social Welfare (MOHSW) to establish incident management system principles. This report describes MOHSW’s Ebola response structure, plans made during the initial assessment, implementation of interventions aimed at improving the system, and plans for further development of the response structure.

To ensure that New York City was prepared to manage Ebola cases and prevent disease transmission, the NYC Department of Health and Mental Hygiene, in close coordination with local hospitals and clinicians, nongovernmental organizations, and city, state, and federal agencies, established systems around Ebola surveillance and management of suspected cases and contacts, and built upon existing general protocols for early recognition and management of persons with febrile illness returning from areas where the epidemic is ongoing.
