Hello Daniel, Emacs is great for programming.  Emacs also has its own
accessibility solutions, which are based on the Emacs user interface
logic and thus give much better results than you could get with a
generic screen reader like Orca.  It is Orca's fault), Emacs is just
quite a specific application.

There are two options:
 - Emacspeak http://emacspeak.sourceforge.net
 - Speechd-el http://www.freebsoft.org/speechd-el

Speechd-el integrates nicely with Orca thanks to Speech Dispatcher.
