Hi there,
As I said in my last post, I agree about survey monkey. Their staff is inconsiderate to boot.
A question about these alternatives, as I am not our web person.
What is the basic setup one would require to use any of them in house?
Sadly he is more of a Windows person with Mac rising. our site also is handled via another company, but if the basics are well basic, it might be an option.

Depending on the nature of your need (a 3rd-party hosted survey
vs. an in-house hosted survey), there are several PHP/Python/Perl
scripts that allow you to host your own survey for free and
adjust them to meet your needs.  A couple that come to mind:


PHP Survey software:

 which became
 Lime Survey:  http://www.limesurvey.org/

 UCCASS:  http://sourceforge.net/projects/uccass/

 and this which I found but isn't free software

 writing one from scratch in Django would be pretty easy

Hope this gives you some alternatives to consider.
