I've always started it from rc.sysinit. There's a comment about "rerunning ourselves under udev" or something (Greek to me). I think I then scanned for USB and after that's set up if I'm feeling especially elegant:

[ -f /bin/brltty ] && /bin/brltty

I don't know whether this really buys you anything over putting it in inittab or not.
