Possible low-tech solution to the signature issue.  If you can produce a
template file which when printed out looks more or less like their form, you
could check (by asking someone?) how much space the file takes up on a printed
page, and then having completed your version of the form by filling in a copy
of your template and printing it out, you could use a straight-edge to
physically sign the printed sheet under where you have already established the
form finishes.

PS:  You may be able to feel where your printed form finishes depending on the
type of printer you use.
