Since this is the first error, odds are good that this one is the root
cause of the others.  Googling a bit for the above error message
brings up a patch by Heinz Mauelshagen that allows log_c to be called
with only 1 parameter.  Without knowing any better, I'd bet my money
on that being the problem.  So you probably need to upgrade your
device-mapper to the version which has Heinz' patch.

Whoops, there's a formatting bug in the device-mapper.  Hm.

No, but at least we've got the right error messages now, so they
should mean something to the developers.  Also, along with the error
messages, you should state the version of the software you're using. 
In this case, copy/paste from 'dmraid --version' and 'dmsetup
version'.
