Bruno's changes for asterisk 1.4 is good, but you need also to add Denis Smirnov's changes for ZAP style addressing - for me it works as a combined version. And RBT may be usefull for Erricson switch only - since for other switches (ZTE and HUAWEI for me) it works without this change fine.
