Apache has defaults that create a directory listing for the case without an index* file, but I'm pretty sure you can change that name with a directive. Should it be "application.xxx"? An index.cgi could look at the environment variables and notice the "application" in the call and treat it specially. A procedure like that would be nearly impossible if there are multiple applications available.
