Because the "Reload" button was hit in Firefox. Try simply to just hit
"Enter" in the URL bar or copy/past the URL into a new tab, you will
find the max-age header is then omitted.

Firefox assumes that when the user hits "reload" he wants everything
as fresh as possible. And most of the time only a small minority of
traffic comes from people hitting "Reload" so you may be able to use
"CacheIgnoreCacheControl Off" and keep most of your performance gains.
