The lack of an error_log entry for a 404 error is rather troublesome.
Are you SURE about that? What you posted above is certainly not a
standard apache access_log format. Are you sure you are using apache?

I bet if you figure out why you aren't getting anything in the
error_log you'll also figure out what the central problem is. Third
party modules? Proxy server?
