You probably have an ErrorDocument which, instead of returning a true 404 error, returns a nice nifty page, with a 200, and then an HTML document that has 404 in it. Dig into Apache ErrorDocument settings and possibly and RewriteRule that involve something like ".*" (any URL at all). Turning on RewriteLog to find out exactly how Apache is re-writing URLs is good for debugging, but can be a problem on a busy site in Production. Good Luck!
