We have a third-party Apache filter to enforce authentication and it turned
out that the filter is the problem as it rejects the request b/c of some
chars in URL. Nothing wrong w/ Apache!

What filter?  mod_security?

Any module that generates a 500 error without writing details to the
error log is broken, as has been demonstrated by the time you had to
waste finding the source of the problem.  You should report the bug to
the source of the software.
