I don't see a single request for a .php file and each one of those requests returend with a successful status code. Based on those logs, there is nothing wrong with your server.

If the server returned a ""The requested operation has failed!" message to your browser then it most definitely put something in the error logs as well. Can I see the log output generated by your failed request?
