In the error log there is this message: [warn] [client 127.0.0.1] [3476] auth_ldap authenticate: user portman authentication failed; URI /ldap/ [ldap_search_ext_s() for user failed][Protocol Error] Using that very same set of configuration directives on a linux host works flawlessly.
