I managed to solve this problem by recompiling Apache from source using the latest release of the Microsoft Platform SDK. Apache still crashes on shutdown, but at least the LDAP authentication seems to be working. It looks like the official Apache Win32 binary must have been built with a very old version of the Platform SDK.
