I've only experience with 2-nodes cluster, with both apache 1.3.x and 2.x. 
Ours are low-cost cluster, i.e without an external storage.
We use drbd for the replication of the data and linux-ha for the heartbeat 
(actually we use this system for other services, like qmail, ..). You can 
find all the information on www.drbd.org and www.linux-ha.org
