As far as I know it means apache is reading the http request from the
client, so it doesn't yet know the URL.

In the vague chance that you are running mod_proxy as a reverse proxy, I
have seen that if the target site it not responding (not down, but just
not responding), then you tend to see a LOT of these, as somehow the mod
proxy gets the request before the scoreboard file(?Is that where the
status data are stored?) is updated.
