I’ve read about the .htaccess file, but the current production server doesn’t seem to have it, but still require a login.

What am I missing on my mac?

3) will be valid only if you're using <location>.

You may be missing modules, but on Linux apache complaints if that's the case.

I would suggest you increase the LogLevel to debug and see what's happening.
