After a quick read through, I don't see anything really odd.
Does anything "interesting" show up in the server logs when this behavior happens?
My first suggestion is that you have a separate access and error log for each vhost (or add hostname logging to the main log). That will make it easier to see where your requests are actually going when they do something you don't expect.
