RewriteCond %{REQUEST_FILENAME} !-f
RewriteCond %{REQUEST_FILENAME} !-d
RewriteRule ^(.*)$ index.cfm?q=$1 [L,QSA]

It sounds like you are looking for the functionality that QSA (Query
String Append) provides.
