to your LogFormat.

Apache distributed by Ubuntu usually has the "vhost_combined" log format already in the config, which should do what you want.

"LogFormat "%v:%p %h %l %u %t \"%r\" %>s %O \"%{Referer}i\" \"%{User-Agent}i\"" vhost_combined"
