This patch fixes the problem.

snd-hda-intel detects the codec properly now:

 cat /proc/asound/cards
 0 [SB             ]: HDA-Intel - HDA ATI SB
                     HDA ATI SB at 0xfe9e8000 irq 18

It also outputs fine. madplay is playing audio fine through both the
front and back panel headers.
