In mid-2003, the US suspended the 2 programs that cover the situation 
you described above, TWOV (Transit WithOut Visa) and ITI (International 
To International), forcing all passengers who would otherwise need a 
visa to enter the US to approach an Embassy overseas to obtain a visa. 
 Like all applications, applicants must pay the $100 machine-readable 
visa fee.  The airline is liable if they board an improperly-documented 
passenger, and so they'll make sure that all passengers who require 
visas will have them.
