Here's my guess about what could happen for the aircraft to get refueled:
1 - the airline wires the money to the fuel handler at ORD
2 - It's possible that the company handling the fuel at ORD may be the same
company that is at DFW so they would just add the ORD fuel to the airline's
bill
3 - The captain would pass around a hat to collect money from the pax to pay
for the fuel (don't laugh - it has happen, though not at a major airport
like ORD)
