If it is not the carrier's website, an agency is issuing the ticket.  That's where they'll send it.  Using the carrier's website, they can give you a call advising you to cease and desist.  Next time, they'll hit your credit card.  That has happened.

They're working on that one.  All they need is a data clearinghouse to which they all subscribe.  And, what has not been mentioned, are the security/privacy issues raised.  Tracking passenger movements does not only have a revenue goal in mind.  Enhanced and data compiled across multiple carriers could be shared with government agencies as well.
